Donation Form

While at PATH on a contract basis I was asked to redesign their existing donation form before a big fund-raising event. They were looking for a form that would catch the eye and draw potential donors in. They also needed to use this on the web as a PDF. So I chose a powerful black and white photo and kept the form simple.
